Adult Programs Transcript Request Procedure
Ben Franklin Career Center follows the federally mandated Family Educational Rights and Privacy Act (FERPA) regulations with regard to the release of any student information. Record requesters must provide a signed request form before any records will be released. Please note that transcripts and/or certificates may not have been kept for all courses taught since 1972; therefore, there is no guarantee that either one can be provided. Transcripts will not be processed for any person who has an outstanding debt to the school.
To request a transcript of academic records a student must:
- Complete and print the Transcript Request Form (below) or pick up a blank form in the main office of Ben Franklin Career Center. All portions of the form must be completed and signed to verify the student’s authorization to release his/her information.
- If you need to obtain your GED records, please go to www.diplomasender.com. We do not have any GED records.
- Submit the completed Transcript Request Form either in person or by mail to: Ben Franklin Career Center, 500 28th Street, Dunbar, WV 25064 Attention: Diana Wood
- Fax to 304-766-0110 OR email to [email protected]
- Pay a $5.00 non-refundable fee for each transcript requested. Cash, check, money orders or online (follow the instructions below in blue print) are accepted via mail or in person. Transcript Requests will not be processed without the appropriate payment.
Adult students may make credit card/debit card payments for their transcripts. Click the link below. Please put your name and program in the cart before checkout. Make sure a transcript request form has been sent also.

5. Requesters may pick up their transcript in person or direct us to mail it to a specific agency/school or individual as indicated on the form. Transcripts will be processed upon receipt of the correctly/fully completed signed request form and payment for pick up or mailing of the records.
